I, Wuya M’bayo of 92 Pipe Line Upper Allen Town in the western Urban of the Republic of Sierra Leone, wish to relinquish and abandon the above name of Wuya M’bayo with immediate effect, and wish to be called, addressed and known, with immediate effect and henceforth, as Goodness Grace M’bayo.
In this regard, I require and authorize all persons at all times to designate, call and address me by the name of Goodness Grace M’bayo, as now registered in volume 155 page 39 of the Administrator and Registrar General’s book.
All documents and records bearing my former name remain valid.
