BY THIS DEED I, the undersigned MOSES NGAIMA of No. 20 Jah Street, Salina Section, Bo City in the Kakua Chiefdom, Bo District in the Southern Province of the Republic of Sierra Leone, a natural born Sierra Leonean subject do hereby absolutely renounce and abandon the use of my former name known as MOSES KULAGBANDA and in lieu thereof assume as from the date hereof the name of MOSES NGAIMA
AND IN PURSUANCE of such name as aforesaid, I hereby declare that I shall at all times hereafter in all Records, Deeds, Instruments in writing and in all actions and proceedings and in all dealings and transactions and upon all occasion whatsoever use and sign the said name of MOSES NGAIMA as my name in lieu of the said name of MOSES KULAGBANDA so renounced as aforesaid.
AND I HEREBY authorize and request all persons to designate and address me by such assumed name of MOSES KULAGBANDA.
IN WITNESS WHERE OF I have hereunto signed my assumed name of MOSES NGAIMA for my relinquished name of MOSES KULAGBANDA and have set my seal this 29 day of March.
